Εισαγωγή: Υπό φυσιολογικές συνθήκες, οι οργανισμοί διατηρούνμια δυναμική παγκρεατική ενδοκρινική μάζα. Τα β παγκρεατικά κύτταραείναι υπεύθυνα για την διατήρηση των επιπέδων γλυκόζης στο αίμααυξάνοντας την έκκριση της ινσουλίνης σε συνθήκες όπως στηνπαχυσαρκία. Ανικανότητα των β κυττάρων να απαντούν στη ζήτηση γιαινσουλίνη προκαλεί υπεργλυκαιμία και Διαβήτη Τύπου 2. Στιςπερισσότερες, ωστόσο, κλινικές περιπτώσεις τα παχύσαρκα άτομα δεναναπτύσσουν διαβήτη καθώς τα νησίδια του Langerhans απαντούν στηναντίσταση στην ινσουλίνη. Δεύτερον, ανθρώπινα παγκρεατικά νησίδια από μηδιαβητικούς και διαβητικούς δότες οργάνων μεταμοσχεύτηκαν στηννεφρική κάψουλα ανοσοανεπαρκών ποντίκων Rag2–/– mice. Ταπειραματόζωα τρέφονταν για 12 εβδομάδες με τροφή χαμηλή ή υψηλή σελιπαρά. Στην διάρκεια των 12 εβδομάδων, καταγράφηκε το βάρος τωνποντικών, τα επίπεδα τριγλυκεριδίων, γλυκόζης και c πεπτιδίου. Μετά τιςθυσίες των πειραματόζωων, το ενδογενές πάγκρεας και το μόσχευμααναλύθηκαν για την ενδοκρινική μάζα, την αναλογία α και β κυττάρων καιτους μηχανισμούς αναγέννησης. Αποτελέσματα : Στην in vivo μελέτη, αύξηση του βάρους τωνποντικών, διπλασιασμός των επιπέδων λίπους και τριγλυκεριδίων καιαύξηση της αντίστασης σε ινσουλίνη την 12 εβδομάδα παρατηρήθηκε σταποντίκια που τρέφονταν με τροφή υψηλής περιεκτικότητας σε λιπαρά. Σταίδια πειραματόζωα, η μακροπρόθεσμη προσαρμογή των ανθρώπινωννησιδίων του Langerhans σε περιβάλλον παχυσαρκίας εξερευνήθηκε.Αρχικά, στοιχεία για την προσαρμογή, την λειτουργία και την έκφραση τωνγονιδίων της ενδοκρινής μάζας in vivo στην παχυσαρκία ανακαλύφθηκαν.Ανάλυση των ανθρώπινων μοσχευμάτων έδειξε λειτουργική προσαρμογήμε αύξηση της έκκρισης του c-πεπτιδίου στο ορό του αίματος,διπλασιασμό της ανθρώπινης ενδοκρινικής μάζας και αύξηση της μάζαςτων β κυττάρων. Έπειτα, ερευνήθηκαν οι μηχανισμοί αναγέννησης τηςενδοκρινικής μάζας και τεχνικές lineage tracing εφαρμόστηκαν για ναπροσδιορίσουν το τύπο των κυττάρων (α ή β κύτταρα) που συμμετέχουνστην αύξηση της μάζα στην παχυσαρκία. Ανάλυση των μηχανισμώναναγέννησης, αποκάλυψε τον πολλαπλασιασμό, την νεογέννηση και τηναποδιαφοροποίηση ως πιθανούς μηχανισμούς αύξησης της ενδοκρινικήςμάζας. Τέλος, αλλαγές στην έκφραση των γονιδίων που συμμετέχουν στοκυτταρικό κύκλο και στο στρες του ενδοπλασματικού δικτύου με τη χρήσημικροσυστοιχιών προσδιορίστηκε για την 6η, 8ηκαι 10ηεβδομάδαπαχυσαρκίας.Συζήτηση: Το συγκεκριμένο πρωτότυπο μοντέλο, μας επιτρέπει γιαπρώτη φορά, την μακροπρόθεσμη μελέτη της προσαρμογής τωνανθρώπινων παγκρεατικών νησιδίων σε περιβάλλον παχυσαρκίας in vivo.Μπορεί να χρησιμοποιηθεί για την διερεύνηση μονοπατιών πουσυμμετέχουν στην αύξηση της μάζας των ανθρώπινων β παγκρεατικώνκυττάρων και στην ταυτοποίηση προδιαθεσικών παραγόντων για τηνδυσπροσαρμοστικότητα των παγκρεατικών νησιδίων του Langerhans.

Secondly, we investigatedthe mechanisms of human islet regeneration and we performed lineagetracing to determine which cell type alpha or beta give rise to the increaseislet mass in obesity. Thirty, in this diet induced obesity model wedeveloped, we looked at the differential gene expression with Affymetrixgene chips in a kinetic study on human islets which were laser capturemicrodissected at 6, 8 and 10 weeks on control or high fat diet.Methods: Archived human pancreatic sections were immunostainedfor endocrine, beta, alpha, fat. In the obese/immunodeficient mouse model,non-diabetic Rag2–/– mice were transplanted under kidney capsule withhuman islets from human brain-deceased donors (non-diabetics donorsand donors with overt metabolic dysfunction). Animals were fed for 12weeks with a control or high-fat diet (HFD), and followed for weight, serum triacylglycerol, fasting blood glucose and human C-peptide. After the micewere killed, human grafts and the endogenous pancreas were analyzed forendocrine volume, distribution of beta and alpha cells, and mechanisms ofregeneration.Results: In the longitudinal study, concomitant with the increasedweight gain, doubling of abdominal fat, increased serum triacylglycerol andreduced insulin sensitivity in 12 week HFD animals we reported thathuman islet grafts showed functional compensation, measured as a morethan doubling of fasting human C-peptide in mouse serum, and histologicaladaptation of islet endocrine mass including increased beta cells. Furtheranalysis of the human grafts revealed proliferation and neogenesis as theresponsible mechanisms for the doubling of the human endocrine mass.Discussion: This novel model allows, for the first time, longitudinalstudies of human islet adaptation to an obese murine environment andmay be instrumental in deciphering pathways involved in human beta cellexpansion, as well as in helping to identify factors predisposing humanbeta cells to undergo decompensation.

Nousavons montré ainsi que les îlots humains non diabétiques s’adaptent invivo à l’obésité en modifiant la masse de cellules b, leur fonction et leurexpression génique. En suite, on a cherché les mécanismes derégénération des ilots humaines (prolifération et néogèneses) et on aidentifié le mécanisme de transdifférenciation des cellules alpha et beta enutilisant la méthode de lineage tracing. Finalement, on a déterminé ladifférence sur l’expression de gène des ilots humains greffé chez les sourissous régime control ou régime riche en graisse en utilisant les pucesd’ARN (Illumina).Methodes et Resultats: Au cours de l’étude longitudinale, dessouris Rag2-/- non diabétiques ont été greffées sous la capsule rénaleavec des îlots humains issus de donneurs en état de mort cérébrale (donneurs non diabétiques ou donneurs avec un dysfonctionnementmétabolique déclaré). Les animaux ont été nourris pendant 2 semainesavec soit un régime contrôle soit un régime riche en graisse (high fat dietHFD). Un suivi du poids, du taux des triglycérides, de la glycémie et du Cpeptidea été mis en place. Après sacrifice des souris, les greffons et lespancréas endogènes ont été analysés pour le volume endocrine, ladistribution des cellules b et a et les mécanismes de régénération descellules pancréatiques. Après 12 semaines sous régime gras, les sourismontraient toutes les caractéristiques typiques de l’obésité, à savoir, uneaugmentation du poids, un doublement de la graisse abdominale, destriglycérides, de la glycémie et une sensibilité à l’insuline réduite. De plus,l’apparition sur ces animaux d’un doublement rapide de la quantité de Cpeptidehumain dans le sérum murin nous indique la mise en place d’unecompensation fonctionnelle. Une analyse histologique des greffons apermis de mettre en évidence une adaptation de la masse endocrine desîlots avec une augmentation des cellules b. D’autres analyses ont identifiéla prolifération et la néogénèse comme les mécanismes responsables dece doublement de la masse endocrine humaine.Discussion: Ce nouveau modèle animal permet d’étudier, in vivosur une longue période, l’adaptation des îlots humains à un environnementobésogène murin. Il peut être utilisé comme un outil dans le décryptagedes voies de signalisation impliquées dans l’expansion des cellules bhumaines et permettre également l’identification des facteurs prédisposantces cellules à subir une décompensation.
