Εισαγωγή: Οι σταφυλόκοκκοι συγκαταλέγονται μεταξύ των μικροοργανισμών που απομονώνονται συχνότερα στο Εργαστήριο Κλινικής Μικροβιολογίας. Εκτός από τον S. aureus, ένας αυξανόμενος αριθμός κοαγκουλάση αρνητικών ειδών, αναδεικνύονται τις τελευταίες δεκαετίες ως αίτια σημαντικών λοιμώξεων καθιστώντας επιτακτική την ορθή ταυτοποίηση του είδους.Σκοπός: Η παρούσα διδακτορική διατριβή εστιάσθηκε στη μελέτη της φαινοτυπικής και μοριακής ταυτοποίησης των κλινικά σημαντικότερων σταφυλοκοκκικών ειδών. Αξιολογήθηκε η απόδοση των, ευρέως διαδεδομένων, ταυτοποιητικών συστημάτων Vitek 2 (bioMérieux) και Phoenix (Becton Dickinson), συγκριτικά με πρότυπη μοριακή μεθοδολογία. Τέλος, μελετήθηκε εργαστηριακά και κλινικά το είδος S. lugdunensis, ως ο πλέον παθογόνος σταφυλόκοκκος μετά τον S. aureus.Υλικό: Χρησιμοποιήθηκαν 206 επιλεγμένα κλινικά στελέχη σταφυλοκόκκων των ειδών S. aureus, S. epidermidis, S. lugdunensis, S. haemolyticus, S. hominis, S. cohnii, S. saprophyticus, S. capitis, S. simulans και S. sciuri, τα οποία ταυτοποιήθηκαν με τις πρότυπες μεθόδους RFLP (restriction fragment length polymorphism) ανάλυσης του γονιδίου tuf ή και προσδιορισμού της αλληλουχίας του γονιδίου 16S rRNA, ενώ το γονίδιο mecA αναζητήθηκε με απλή PCR. Ως μάρτυρες συμπεριλήφθηκαν, 17 τυχαία επιλεγμένα κλινικά στελέχη άλλων γενών και 20 πρότυπα στελέχη.Μέθοδοι: Τα κλινικά στελέχη των σταφυλοκόκκων ταυτοποιήθηκαν φαινοτυπικά με τα συστήματα Vitek 2 και Phoenix. Για το τελευταίο δοκιμάστηκε και το νεότερο πρωτόκολλο του εναιωρήματος θολερότητας 0,25 McFarland. Η στατιστική σημαντικότητα των αποτελεσμάτων σύγκρισης των μεθόδων προσδιορίστηκε με τη δοκιμασία χ2. Αναπτύχθηκε πρωτόκολλο αναγνώρισης του S. lugdunensis με απλή PCR επιλέγοντας το γονίδιο fbl, και πρωτόκολλο πολυπλεκτικής αντίδρασης στοχεύοντας τα γονίδια femA (S. aureus και S. saprophyticus), fbl (S. lugdunensis), sodA (S. haemolyticus, S. capitis και S. simulans), ένα χρωμοσωμικό τμήμα άγνωστης κωδικής σημασίας (S. epidermidis), τμήμα του 16S rRNA (S. hominis) και το γονίδιο mecA (αντοχή στη μεθικιλλίνη). Για την απευθείας εφαρμογή της πολυπλεκτικής αντίδρασης σε φιάλες θετικών αιμοκαλλιεργειών δοκιμάστηκαν πέντε πρωτόκολλα απομόνωσης βακτηριακού γενετικού υλικού (έκπλυσης με αλκαλικό διάλυμα και θερμικής λύσης, έκπλυσης με απεσταγμένο νερό, έκπλυσης με BSA, οργανικής εκχύλισης με βενζυλική αλκοόλη και θειοκυανιούχο γουανιδίνη και το High Pure PCR Template Preparation Kit [Roche]). Τέλος, για τη μελέτη του είδους S. lugdunensis αναζητήθηκαν αναδρομικά τα στελέχη που είχαν απομονωθεί στη διάρκεια μιας τριετίας στο Εργαστήριο Κλινικής Μικροβιολογίας του Πανεπιστημιακού Γενικού Νοσοκομείου «ΑΤΤΙΚΟΝ». Μετά την οριστική ταυτοποίησή τους (προσδιορισμός αλληλουχίας 16S rRNA), τα στελέχη εξετάστηκαν με τα συστήματα Phoenix και API Staph (bioMérieux), καθώς και με απλές βιοχημικές δοκιμασίες, ελέγχθηκε η ευαισθησία τους, αναζητήθηκε η κλινική τους σημασία και προσδιορίστηκε η γενετική τους ποικιλομορφία (μέθοδος PFGE [pulsed-field gel electrophoresis]).Αποτελέσματα: Τα ποσοστά σωστής ταυτοποίησης για τον S. aureus, τον S. epidermidis και τα υπόλοιπα κοαγκουλάση αρνητικά είδη υπολογίστηκαν στο 100%, 100% και 93,3% για το Vitek 2, στο 100%, 86% και 82,2% για το καθιερωμένο πρωτόκολλο του Phoenix και στο 100%, 92% και 82,2% για το πρωτόκολλο του εναιωρήματος των 0,25 McFarland, αντίστοιχα. Αναφορικά με το σύνολο των κοαγκουλάση αρνητικών ειδών, το σύστημα Vitek 2 υπερείχε και των δύο πρωτοκόλλων του Phoenix (p=0,002 και p=0,007, αντίστοιχα), ενώ η σύγκριση των τελευταίων μεταξύ τους δεν ανέδειξε διαφορά (p=0,467). H PCR που σχεδιάστηκε για την αναγνώριση του S. lugdunensis ανίχνευσε το γονίδιο fbl μόνο στα στελέχη του είδους, ενώ και η πολυπλεκτική αντίδραση πολλαπλασίασε ειδικά τα αναμενόμενα προϊόντα από όλα τα κλινικά και πρότυπα στελέχη και προσδιόρισε ορθά την αντοχή τους στη μεθικιλλίνη. Η απευθείας εφαρμογή της πολυπλεκτικής PCR σε υλικό θετικών αιμοκαλλιεργειών κατέστη δυνατή μόνο με την οργανική μέθοδο εκχύλισης βακτηριακού DNA. Αναφορικά με τον S. lugdunensis, η ταυτοποίηση 14 στελεχών του είδους αποδείχθηκε προβληματική με το σύστημα Phoenix. Τα ποσοστά αντοχής στα αντιμικροβιακά δεν ξεπέρασαν το 21% για την πενικιλλίνη και το 7% για τη μεθικιλλίνη, τη γενταμικίνη και τις κινολόνες. Τουλάχιστον στα μισά περιστατικά ο S. lugdunensis συνδέθηκε με κλινικά σημαντικές λοιμώξεις, εκ των οποίων τα δύο περιστατικά ενδοκαρδίτιδας παρουσίασαν άριστη έκβαση. Η PFGE κατέδειξε χαμηλή γενετική ποικιλομορφία για το είδος. Συμπεράσματα: Η ορθή ταυτοποίηση των κοαγκουλάση αρνητικών σταφυλοκόκκων χρησιμοποιώντας τα αυτοματοποιημένα φαινοτυπικά συστήματα δεν είναι δεδομένη. Η κλινική εφαρμογή μοριακών τεχνικών ταυτοποίησης, όπως αυτές που αναπτύχθηκαν στην παρούσα μελέτη, αναμένεται να βελτιώσει την αναγνώριση του είδους των κοαγκουλάση αρνητικών σταφυλοκόκκων συμβάλλοντας στον ακριβέστερο προσδιορισμό του παθογενετικού τους ρόλου.

A single-step PCR assay for S. lugdunensis identification, as well as a multiplex PCR protocol for the concomitant detection of eight common species (S. aureus, S. epidermidis, S. hominis, S. haemolyticus, S. saprophyticus, S. lugdunensis, S. capitis και S. simulans) and part of the mecA gene were developed. Finally, the microbiological and clinical characteristics of S. lugdunensis, deemed the most pathogenic species after S. aureus, were analysed. Materials: A total of 206 selected clinical staphylococcal isolates, representing S. aureus, S. epidermidis, S. lugdunensis, S. haemolyticus, S. hominis, S. cohnii, S. saprophyticus, S. capitis, S. simulans and S. sciuri species, were studied. The isolates were identified by means of tuf gene RFLP analysis and/or 16S rRNA sequencing, while mecA PCR was used to determine their resistance towards methicillin. Seventeen randomly selected non-staphylococcal clinical isolates of various genera, as well as 20 reference strains served as controls.Methods: Clinical staphylococcal isolates were phenotypically identified by both Vitek 2 and Phoenix systems. For the latter, the novel 0,25 McFarland inoculum procedure was also examined. The χ2 test for paired observations was performed for between-methods comparisons. A simple PCR protocol for S. lugdunensis identification using fbl gene as a species-specific marker, as well as a multiplex PCR assay targeting femA gene (for S. aureus and S. saprophyticus), fbl gene (for S. lugdunensis), sodA gene (for S. haemolyticus, S. capitis and S. simulans), a chromosomal fragment of unknown coding potential (for S. epidermidis), part of 16S rRNA gene (for S. hominis) and mecA gene (for methicillin resistance) were designed. For the application of the multiplex PCR directly from positive blood cultures, five extraction procedures (a rapid alkali wash and heat lysis method, two protocols using either sterile water or BSA to wash the bacterial cell pellet, an organic extraction procedure using benzyl alcohol and guanidine thiocyanate and the High Pure PCR Template Preparation Kit [Roche]) were tested. Finally, in an attempt to study S lugdunensis species, all S. lugdunensis isolates recovered during a three-year period in the Clinical Microbiology Laboratory of ‟ATTIKON” University hospital were retrospectively analysed. Following their definitive identification (by 16S rRNA gene sequencing), the isolates were examined by Phoenix and API Staph (bioMérieux) systems, as well as by simple biochemical tests. Their antimicrobial susceptibility profiles, clinical significance and genetic diversity (by PFGE) were also assessed.Results: Correct identification rates for S. aureus, S. epidermidis and the remaining coagulase-negative species reached 100%, 100% and 93,3% for Vitek 2, 100%, 86%, 82,2% for the 0,5 McFarland Phoenix protocol and 100%, 92% and 82,2% for the novel 0,25McFarland Phoenix, protocol respectively. Regarding the identification of all coagulase negative species, Vitek 2 was significantly more accurate in comparison to both Phoenix protocols (p=0,002 and p=0,007, respectively), while between-protocols comparison revealed no significant difference (p=0,467). The single-step PCR method designed for S. lugdunensis identification amplified the fbl gene only from S. lugdunensis isolates, while the multiplex reaction yielded the expected species-specific products from all clinical and reference strains and determined accurately their resistance towards methicillin. The application of the multiplex protocol directly from positive blood cultures was feasible only by the organic bacterial DNA extraction procedure. Regarding S. lugdunensis, the identification of the 14 isolates belonging to the species proved to be problematic by Phoenix system. Antimicrobial resistance rates did not exceed 21% for penicillin and 7% for methicillin, gentamicin and fluoroquinolones. S. lugdunensis was associated with clinically significant infections in at least half of the cases (among them two patients with endocarditis had excellent clinical outcome). PFGE showed S. lugdunensis to be of low genetic diversity.Conclusions: Correct species-level identification of coagulase-negative staphylococci by automated phenotypic systems cannot be taken for granted. Clinical application of molecular techniques, like the ones developed in the present thesis, is expected to improve the assignment of coagulase-negative isolates to the correct species, assisting in the full elucidation of their pathogenic potential.
