Οι πυρετικοί σπασμοί είναι καλοήθης κατάσταση που όμως προβληματίζει και ανησυχεί τους γονείς. Ειδικά το πρώτο επεισόδιο απαιτεί την εισαγωγή του ασθενούς στο νοσοκομείο και τη διερεύνησή του. Οι ιογενείς λοιμώξεις παίζουν καθοριστικό ρόλο στην αιτιολογία των πυρετικών σπασμών με κύριο παθογενετικό μηχανισμό την πρόκληση πυρεξίας. Για κάθε άτομο υπάρχει συγκεκριμένος ουδός θερμοκρασίας σώματος για την εκδήλωση πυρετικών σπασμών που καθορίζεται από γενετικούς και περιβαλλοντικούς παράγοντες, τροποποιείται με τη ηλικία και την εγκεφαλική ωρίμανση και επηρεάζεται από μεταβολικές διαταραχές που συχνά συνοδεύουν τον πυρετό. Ο τύπος των πυρετικών σπασμών απλών ή σύνθετων σχετίζεται με την ανοσολογική απάντηση στη λοίμωξη και την παραγωγή κυτοκινών. Ορισμένοι ιοί έχουν νευροτρόπο δράση και διεισδύοντας στο κεντρικό νευρικό σύστημα προκαλούν εγκεφαλοπαθητική νόσηση που οδηγεί στην εκδήλωση κυρίως επιπεπλεγμένων πυρετικών σπασμών. Επίσης ορισμένοι ιοί μπορεί να προκαλούν αναζωπύρωση άλλων ιών που βρίσκονται σε λανθάνουσα κατάσταση στο κεντρικό νευρικό σύστημα. Με βάση την πιο πάνω μελέτη που αφορά σε πληθυσμό που εξυπηρετείται από μία από τις τέσσερις παιδιατρικές κλινικές του λεκανοπεδίου Αττικής: • Η πλειοψηφία των ασθενών ήταν ηλικίας < 2 ετών (48/65 = 73,8%), εύρημα συμβατό με το ότι η πρωτολοίμωξη συμβαίνει στο μεγαλύτερο ποσοστό ως την ηλικία των 24 μηνών. • Ο HHV-6 B ήταν το στέλεχος που κυρίως απομονώθηκε και θεωρείται παγκοσμίως ότι συσχετίζεται με την εμφάνιση πυρετικών σπασμών. • Το ? των παιδιών που εισάγονται προς διερεύνηση λόγω πρώτου επεισοδίου πυρετικών σπασμών στο νοσοκομείο εμφανίζουν πρωτολοίμωξη από ερπητοϊό 6, ποσοστό από τα υψηλότερα στη διεθνή βιβλιογραφία. Η ανίχνευσή του με μοριακές και ορολογικές μεθόδους, ως εξέταση ρουτίνας για τα παιδιά αυτά θα ήταν δόκιμη. Με τη μέθοδο της real time PCR, η ταχεία ανίχνευση του ιού θα συμβάλλει στην αποφυγή άλλων διαγνωστικών εξετάσεων και στη μείωση της κατάχρησης των αντιμικροβιακών φαρμάκων. • Με βάση τα ευρήματα της παρούσας μελέτης η πρωτολοίμωξη από τον ιό δεν προδιαθέτει σε εμφάνιση υποτροπών πυρετικών σπασμών αλλά ούτε και σύνθετων σπασμών αλλά το ερώτημα αυτό όπως και το κατά πόσον η πρωτολοίμωξη είναι προδιαθεσικός παράγοντας για την εμφάνιση επιληψίας είναι ακόμη υπό διερεύνηση. • Για τη διάγνωση της πρωτολοίμωξης από ερπητοϊό 6 στα παιδιά υπάρχει συμφωνία των ερευνητών ότι η ανεύρεση ιικού DNA στον ορό αίματος ή το πλάσμα είναι επιτυχής για τη διάγνωση της πρωτολοίμωξης στα παιδιά. • Η μοριακή μέθοδος που εφαρμόστηκε στην παρούσα μελέτη (real time PCR με τη χρήση του συστήματος Light Cycler 2.0) εμφανίζει υψηλή ειδικότητα και ευαισθησία αλλά πρέπει να πραγματοποιείται σε σύντομο χρονικό διάστημα, με μέγιστο τους 6 μήνες από τη συλλογή των δειγμάτων λόγω μη σταθερότητας του ιού στις συνθήκες συντήρησής του για μεγαλύτερο χρονικό διάστημα. • Στα παιδιά που εμφανίζουν πυρετικούς σπασμούς η διάγνωση της πρωτολοίμωξης δεν έχει να προσφέρει θεραπευτικά, είναι όμως ένα σημαντικό βήμα στη διάγνωση του αιτίου των σπασμών, ενημέρωση και καθησυχασμό των γονέων. Επίσης συμβάλλει στην ορθολογική χρήση των αντιβιοτικών όπως προαναφέρθηκε. Συμπερασματικά η μελέτη της μοριακής βιολογίας του ερπητοϊού 6 (όπως και άλλων νευροτρόπων ερπητοϊών) είναι σημαντική όχι μόνο από την πλευρά της βασικής επιστημονικής έρευνας αλλά και για την καλύτερη κατανόηση των μηχανισμών που οδηγούν στην εμφάνιση των πυρετικών σπασμών. Η παρούσα μελέτη είναι η πρώτη στον ελλαδικό χώρο, που διευρύνει τη γνώση για τον HHV-6, έναν ιό με παγκόσμια κατανομή αλλά ποικίλες βιολογικές ιδιότητες και κλινικές εκδηλώσεις.

After primary infection the virus remains latent not only in saliva, but also in CNS and may occasionally reactivate systematically under immunodeficient status. In immunocompetent hosts HHV-6 may reactivate during pregnancy or with HHV-7, measles or dengue infections. In recent years attention has focused on febrile seizures due to HHV-6 infection. Febrile seizures are an age depended condition occurring in children from 6 months to 5 years old. The current belief is that HHV-6 plays a causative role in FSs, based on the neurotropic properties of this virus and the similarity of the group in which febrile seizures and HHV-6 infection occur. In order to investigate the frequency and clinical characteristics of primary HHV-6 infection of hospitalized children with febrile seizures in our geographic region, we examined children (6 months to 5 years) without known neurological disease, for primary HHV-6 infection, by qualitative real-time PCR in acute phase plasma and anti-HHV-6 antibody titers in acute and convalescent (2-4 weeks later) serum by using indirect immunoflorescent assay (IFA). From November 2002 until December 2005, ninety-eight consecutive children aged 6 months to 5 years that were hospitalized in a tertiary Children’s hospital, for febrile seizures without known neurological disease were evaluated and examined for primary HHV-6 infection. Sixty-five of these children returned 2-4 weeks later for collection of convalescent-phase specimens. A standard questionnaire was given to the parents and they were informed about the purposes of the study. The history and clinical findings were recorded on a standard form as well as the diagnosis made at discharge. For the purpose of the study, the case definition of febrile seizures was a convulsive seizure in infants and children in association with fever, but without evidence of any definitive causative disease, such as CNS infection, metabolic abnormality or intoxication. Thus, both simple and complicated seizures were included. None of the patients had a history of febrile seizures. In addition to routine laboratory data (complete blood cell count, urinalysis, electrolyte determinations, cultures), blood samples were also collected and tested by qualitative real-time PCR in acute phase plasma and anti-HHV-6 antibody titers in acute and convalescent (2-4 weeks later) serum by using indirect immunoflorescent assay (IFA), from patients who agreed to be enrolled in the study. Whenever lumbar puncture was performed, CSF was also collected. HHV-6 DNA was extracted from blood samples using the commercially available High Pure Nucleic Acid kit (Roche) according to manufacture’s instructions. Real time PCR in blood sera was done by using the Light Cycler System 2. 0 as described elsewhere and the antibody titers to ΗHV-6 were measured by indirect immunofluorescence assay using the commercially available Biognost antibody Assay (Bios Gmbh, Mϋnchen, info@ bios-world. com). Eighty-five children hospitalized for elective surgery during the same time. period at the same hospital served as controls. A total of 65 children with febrile seizures were enrolled in the study, 36 were boys and 29 girls, mean age 21.35 months. Fifty-five patients had experienced febrile seizures for the first time and 10 had experienced a second episode. Primary HHV-6 infection was verified in 13 (13/65 = 20%) children. Mean age of these HHV-6 patients was 18.54 months. Ten of them had a positive for HHV-6 DNA by real time PCR in serum, while the remaining three were diagnosed as having HHV-6 primary infection by serology. HHV-6 DNA was not detected in three available CSF samples. Eight strains HHV-6 were identified by PCR as type B and two as type A. None of the controls had primary HHV-6 infection while 49.4% had IgG anti-HHV-6 antibodies. All children with primary HHV-6 infection had febrile seizures for the first time (13/55 = 23.6%). Six out of 13 (46.1%) HHV-6 positive children had typical roseola one of which was complicated with acute otitis media, 6 presented with respiratory or gastrointestinal infection, and one (7.2%) had no other symptoms than fever and seizures. Three of 13 children with primary HHV-6 infection and 8 of 52 children without, had complicated seizures (P, not significant). In the present study, it was observed that the two main clinical characteristics of children with primary HHV-6 infection were exanthema subitum and FSs. The majority of the HHV-6 infections were caused by type B viruses as expected, and only two infections were caused by HHV-6 type A viruses. The association between HHV-6 and FSs has been reported previously by others. Although in the present report a cause and effect relationship between HHV-6 and FS could not be established, all children with primary HHV-6 infection experienced their first episode of FSs. Moreover, in 24% (13/55) of children with a first episode of FSs the seizures were attributed to primary HHV-6 infection. On the contrary, the second episode of FSs was not associated with primary HHV-6 infection as has been noted by others [9]. Several studies have examined the role of primary HHV-6 infection in FSs. Among children with first episode of FSs primary HHV-6 was considered the cause of seizure in 26.2 to 42.8% of children, a percentage similar to that found in our study. On the other hand, 8 to 20% of children with primary HHV-6 infection had as a protean manifestation FSs. In contrast to those studies, all of our children (13/13) with primary HHV-6 infection experienced febrile seizures. This difference between our study and previous reports could be attributable to the different methodology, study design, and heterogeneity of the study populations. Moreover, virus strains isolated from diverse geographic location may have different biological properties. Notably, the majority of infections in the present study were caused by type B virus which is more frequently associated with febrile seizures. Although primary HHV-6 infection is found with high frequency among children with febrile seizures, whether a causal association truly exists remains to be proven. A case-control study of 35 children with FSs and 33 controls, did not show an association between HHV-6 and febrile seizures Nevertheless, an increasing number of studies support their association. Thus, in the medical literature it is considered that primary HHV-6 infection is an important factor in the pathogenesis of febrile seizures. In conclusion, our findings indicate that primary HHV-6 infection is frequently associated with febrile seizures in young children in our geographic region, and should be taken into consideration, especially in patients experiencing their first episode. The fact that 20% of the children seen in the emergency department with febrile seizures experience primary HHV-6 infection, supports the inclusion of HHV-6 detection in routine laboratory tests. Primary HHV-6 infection is not a risk for recurrent FSs.
