Η νόσος Alzheimer (AD) αποτελεί μία από τις πλέον κοινές νευροεκφυλιστικές διαταραχές παγκοσμίως. Χαρακτηρίζεται από απώλεια μνήμης και έκπτωση των γνωσιακών λειτουργιών, συμπτώματα που εμφανίζονται σε ήπια μορφή αρχικά και επιδεινώνονται δραματικά με το πέρασμα του χρόνου. Η θεραπευτική αντιμετώπιση της νόσου περιλαμβάνει τη χορήγηση κυρίως αναστολέων της ακετυλο-χολινεστεράσης, οι οποίοι επιτυγχάνουν απλώς επιβράδυνση της εξέλιξης των συμπτωμάτων. Βασική αιτία της αναποτελεσματικής θεραπευτικής αντιμετώπισης αποτελεί ο πολυ-παραγοντικός χαρακτήρας της ασθένειας. Κύρια ανατομικά χαρακτηριστικά της νόσου Alzheimer είναι η εναπόθεση β-αμυλοειδών πλακών και ο σχηματισμός νευροϊνωδών κόμβων (NFTs). Χαρακτηρίζεται από απώλεια μνήμης και έκπτωση των γνωσιακών λειτουργιών, συμπτώματα που εμφανίζονται σε ήπια μορφή αρχικά και επιδεινώνονται δραματικά με το πέρασμα του χρόνου. Η θεραπευτική αντιμετώπιση της νόσου περιλαμβάνει τη χορήγηση κυρίως αναστολέων της ακετυλο-χολινεστεράσης, οι οποίοι επιτυγχάνουν απλώς επιβράδυνση της εξέλιξης των συμπτωμάτων. Βασική αιτία της αναποτελεσματικής θεραπευτικής αντιμετώπισης αποτελεί ο πολυ-παραγοντικός χαρακτήρας της ασθένειας. Έχει βρεθεί ωστόσο ότι το οξειδωτικό στρες, αλλά και η φλεγμονή αποτελούν σημαντικούς παράγοντες που διαδραματίζουν πρωτεύοντα ρόλο στην εξέλιξη της ασθένειας, επάγοντας τόσο τη συσσώρευση των αμυλοειδών πεπτιδίων, όσο και την υπερφωσφορυλίωση της πρωτεΐνης ταυ, οδηγώντας σε δημιουργία NFTs. Ιδιαίτερη έμφαση έχει δοθεί στον σχεδιασμό πολυδραστικών μορίων, τα οποία είναι σε θέση να δρουν σε περισσότερους στόχους που σχετίζονται με την AD. Στο πλαίσιο αυτής της προσέγγισης σχεδιάσαμε και συνθέσαμε ενώσεις, οι οποίες αποτελούνται από ένα τμήμα που αναμένεται να προσδίδει αντιοξειδωτικές δράσεις και ένα τμήμα, το οποίο αναμένεται να προσδίδει δράση στο ΚΝΣ. Οι ενώσεις που προέκυψαν αποτελούν αμίδια φαινολικών οξέων με γνωστή αντιοξειδωτική ικανότητα με αμίνες, όπως η προλίνη, το GABA, η γκαμπαπεντίνη και το νιπεκοτικό οξύ, οι οποίες εκδηλώνουν δράση στον εγκέφαλο. Όσον αφορά τη βιολογική δραστικότητα, εξετάστηκε η αντιοξειδωτική δράση των ενώσεων, καθώς και η ικανότητά τους να αναστέλλουν τη γλυκοζυλίωση πρωτεϊνών, διαδικασία η οποία οδηγεί σε νευροτοξικά τελικά προϊόντα γλυκοζυλίωσης που εμφανίζονται σε ιδιαίτερα υψηλές συγκεντρώσεις κατά την AD. Παράλληλα εξετάστηκε και η ανασταλτική δράση τους επί της ακετυλο-χολινεστεράσης, ενώ θεωρήθηκε σκόπιμο να μελετηθεί και η αντιφλεγμονώδης δράση, η οποία εκφράστηκε ως ανασταλτική δράση επί του οιδήματος που παράγεται από καρραγενίνη. Τέλος, έγινε προσπάθεια εκτίμησης της ικανότητας των ενώσεων να διέρχονται μέσω του αιματοεγκεφαλικού φραγμού. Επιβεβαιώθηκε ότι η παρουσία φαινολικού υδροξυλίου στις ενώσεις προσδίδει σημαντική αντιοξειδωτική δράση. Ενώσεις με αντιοξειδωτική δράση αναστέλλουν σε σημαντικό βαθμό και τη γλυκοζυλίωση πρωτεϊνών που πραγματοποιείται με τη βοήθεια φρουκτόζης. Επιπλέον μερικές ενώσεις βρέθηκαν να αναστέλλουν σε σημαντικό βαθμό την ακετυλο-χολινεστεράση εμφανίζοντας τιμές IC50<50μΜ. Αναφορικά με την αντιφλεγμονώδη τους δράση, οι ενώσεις εμφάνισαν ικανοποιητική έως πολύ καλή ανασταλτική δράση επί του παραγόμενου από καρραγενίνη οιδήματος. Μάλιστα η αντιοξειδωτική ικανότητα των ενώσεων οδήγησε και αυξημένη αντιφλεγμονώδη δράση. Τέλος, όπως προέκυψε από μελέτες, οι οποίες βασίστηκαν στον υπολογισμό φυσικοχημικών ιδιοτήτων, όπως η λιποφιλικότητα και η συνολική πολική επιφάνεια των ενώσεων, οι τιμές logBB κυμαίνονται σε τέτοια πλαίσια, που υποδεικνύουν μέτρια έως υψηλή κατανομή στο ΚΝΣ. Τα παραπάνω ευρήματα επιβεβαιώνουν τον αρχικό επιτυχή σχεδιασμό των ενώσεων. Προβλέπεται ότι ενώσεις με δυνατότητα κατανομής εντός του ΚΝΣ, οι οποίες εμφανίζουν τις παραπάνω δράσεις, θα μπορούσαν να χρησιμοποιηθούν ως ενώσεις-οδηγοί για την περαιτέρω έρευνα σχετικά με την ανακάλυψη πολυδραστικών ενώσεων ενάντια στην AD.

In addition to senile plaques and NFTs, oxidative stress has been found to play a key role in the development of AD. Large amounts of oxygen are consumed in the brain, therefore high concentrations of Reactive Oxygen Species (ROS) are produced there. Due to low antioxidant protection of the Central Nervous System (CNS), oxidative stress is easily established, resulting in lipid peroxidation, protein oxidative damage, leading to cellular death and neurodegeneration. Moreover, oxidative stress induces β-amyloid peptide aggregation and NFT formation and leads to the production of Advanced Glycation End-products (AGEs), that are characterized by neurotoxicity. Inflammation is another key factor contributing to AD development. Microglia activation leads to immune-response and subsequently to the production of inflammation mediators, such as cytokines and prostaglandins. Inflammatory enzymes such as lipoxygenases (LOX) and cyclooxygenases (COX) are overexpressed during AD, resulting in general inflammatory reactions causing apoptosis and neurodegeneration. As far as neurotransmitters are concerned, acetylcholine is found in very low concentrations, while NMDA receptors are overactive, leading to excitotoxicity. Moreover, GABAergic neurotransmission is affected, contributing to behavioral dysfunctions during AD. Considering the multi-factorial character of AD, we have designed and synthesized molecules, which combine more than one pharmacophores. Such compounds could be able to act at various targets associated with AD and thus achieving more effective treatment of the disease. We have used carboxylic acids with antioxidant activity (butylated hydroxy-benzoic acid, butylated hydroxy-cinnamic acid, ferulic acid, sinapic acid, trolox) which were amidated with moieties that could act in the CNS. Furthermore, we have used carboxylic acids without antioxidant characteristics in order to compare other actions of the compounds with the respective actions of compounds with simultaneous potential. Amines we used for the synthesis were the following: •Proline methyl-ester: proline includes the pharmacophore of nootropic drugs (piracetam, aniracetam) •GABA methyl-ester: GABA is the main inhibitory neurotransmitter in brain •Gabapentin methyl-ester: gabapentin decreases the synaptic function by inhibiting Ca2+ and Na+ channels •Nipecotic acid ethyl-ester: nipecotic acid is a GABA reuptake inhibitorAll compounds were purified by flash column chromatography and identified by spectroscopic techniques (1H-NMR, 13C-NMR). The synthesized compounds were tested for their antioxidant potential through their inhibitory effect on lipid peroxidation and their ability to reduce the stable free radical DPPH. In addition, their activity against protein glycation induced by reducing sugars (fructose) was examined. We also tested their inhibitory activity against AchE and their effect on paw oedema produced by carrageenan in rats. Finally, we tried to estimate the ability of our compounds to penetrate Blood-Brain-Barrier (BBB), as they are supposed to act in the CNS.Compounds with antioxidant moieties and high lipophilicity proved to possess very good antioxidant activity, both as lipid peroxidation inhibitors and as DPPH scavengers. Trolox derivatives proved to be the most active antioxidant compounds with IC50 values lower than 10μΜ. Compounds with antioxidant potential inhibited the formation of AGEs significantly (35-56% at 1mM). Additionally, some of our compounds had moderate to very good inhibitory activity against AchE. Nipecotic acid derivatives were the most promising inhibitors (IC50 values lower than 70μΜ). All compounds inhibited the carrageenan-induced oedema in rats, especially the ones with significant antioxidant activity. Finally, considering ClogP and TPSA values of our molecules, all of them are supposed to reach the CNS adequately. Gabapentin derivatives are expected to be distributed into the brain highly, due to their increased lipophilicity and low TPSA. Our results confirm our successful initial design, since our compounds can act against various targets related to AD and can penetrate the Blood-Brain-Barrier. Therefore, they could be used as lead compounds for further investigation and optimization for the development of multi-functional molecules against Alzheimer’s Disease.
