Εισαγωγή: Παγκοσμίως, σημαντικός αριθμός επιζώντων καρκίνου μαστού έχει ανήλικα παιδιά. Τα παιδιά αυτά μπορεί να νιώθουν άγχος, θλίψη, φόβο, κατάθλιψη ή να παρουσιάσουν καλύτερη σχετιζόμενη με την υγεία ποιότητα ζωής και βελτιωμένες κοινωνικές/σχολικές επιδόσεις. Οι διαταραχές συναισθήματος και συμπεριφοράς μπορεί να συνεχιστούν στην ενήλικη ζωή. Στόχος της μελέτης είναι η διερεύνηση τέτοιων διαταραχών και παραγόντων εμφάνισής τους, σε παιδιά, που η μητέρα τους έχει διαγνωσθεί με καρκίνο μαστού, στον ένα μήνα και στον ένα χρόνο μετά το χειρουργείο της μητέρας. Τα παιδιά αυτά μπορεί να νιώθουν άγχος, θλίψη, φόβο, κατάθλιψη ή να παρουσιάσουν καλύτερη σχετιζόμενη με την υγεία ποιότητα ζωής και βελτιωμένες κοινωνικές/σχολικές επιδόσεις. Οι διαταραχές συναισθήματος και συμπεριφοράς μπορεί να συνεχιστούν στην ενήλικη ζωή. Στόχος της μελέτης είναι η διερεύνηση τέτοιων διαταραχών και παραγόντων εμφάνισής τους, σε παιδιά, που η μητέρα τους έχει διαγνωσθεί με καρκίνο μαστού, στον ένα μήνα και στον ένα χρόνο μετά το χειρουργείο της μητέρας. Μεθοδολογία: Μητέρες (n=164) ανήλικων παιδιών, οι οποίες έχουν διαγνωστεί με καρκίνο μαστού, ένα μήνα και ένα χρόνο μετά το χειρουργείο συμπλήρωσαν κλίμακες για: (α) ένα από τα παιδιά τους: Child Behavior Checklist (CBCL/1,5-5 & CBCL/6-18), (β) τις ίδιες: Symptom Checklist-90-R (SCL90-R), Body Image Scale (BIS), 12-Item Health Survey (SF-12 v2®), Ways of Coping Checklist, Defense Style Questionnaire (DSQ) και (γ) την οικογένεια: Family Assessment Device-General Functioning Subscale (FAD-GFS), Family Crisis Oriented Personal Evaluation Scales (F-COPES). Αποτελέσματα: Η μέση ηλικία του δείγματος ήταν για τα παιδιά 11,1 ± 4,5 έτη και για τις μητέρες 44 ± 5,5 έτη. Το 52,4% των παιδιών ήταν αγόρια. Το 54,9% των μητέρων υποβλήθηκε σε μαστεκτομή. Η πλειοψηφία των συμμετεχουσών, τόσο στην πρώτη όσο και στη δεύτερη μέτρηση, είχαν μιλήσει οι ίδιες στο παιδί για την ασθένεια (83,4% και 84,2%, αντίστοιχα) και τη μαστεκτομή (60,7% και 65,5%, αντίστοιχα). Στη δεύτερη αξιολόγηση παρατηρήθηκε αύξηση (10,8%) των πληροφοριών που παρείχαν οι μητέρες στο παιδί για την ασθένεια. Επίσης, σημειώθηκε αύξηση (3,9%) στα Εσωτερικευμένα, μείωση (2,2%) στα Εξωτερικευμένα και αύξηση (1,2%) στα Συνολικά Προβλήματα των παιδιών. Μεταξύ πρώτης και δεύτερης καταγραφής, τα Εσωτερικευμένα Προβλήματα των παιδιών παρουσίαζαν μεγαλύτερη συχνότητα (14,2% και 18,1% αντίστοιχα) σε σχέση με τα Εξωτερικευμένα (11,6% και 9,4% αντίστοιχα), ενώ δεν διαπιστώθηκαν στατιστικά σημαντικές αλλαγές στη μέση τιμή των κλιμάκων του «CBCL». Σύμφωνα με τις πολυμεταβλητές αναλύσεις, ο Γενικός Δείκτης Συμπτωμάτων (SCL-90) της μητέρας σχετίζεται με Εσωτερικευμένα (P=0,012), Εξωτερικευμένα (P=0,036) και Συνολικά Προβλήματα (P=0,001) στα παιδιά (CBCL) στην πρώτη μέτρηση, ενώ το Σύνολο Θετικών Συμπτωμάτων (SCL-90) σχετίζεται με Εσωτερικευμένα (P=0,018) στη δεύτερη. Η Θετική Προσέγγιση (Ways of Coping) της μητέρας σχετίζεται αρνητικά με Εσωτερικευμένα Προβλήματα (P=0,046) στα παιδιά στην πρώτη μέτρηση. Η Διεκδικητική Επίλυση Προβλήματος (Ways of Coping) σχετίζεται με Εσωτερικευμένα (P=0,001), Εξωτερικευμένα (P=0,000) και Συνολικά (P=0,001) Προβλήματα στην πρώτη μέτρηση και με Εξωτερικευμένα (P=0,031) στη δεύτερη. Το Προφίλ Αυτοθυσίας (DSQ) της μητέρας σχετίζεται με Εσωτερικευμένα (P=0,034) στην πρώτη και Εσωτερικευμένα (P=0,030) και Συνολικά (P=0,004) Προβλήματα στα παιδιά, στη δεύτερη μέτρηση, ενώ το Δυσπροσαρμοστικό Προφίλ (DSQ) σχετίζεται με Εξωτερικευμένα Προβλήματα (P=0,006) στη δεύτερη. Υψηλότερες τιμές στο FAD-GFS σχετίζονται με Εσωτερικευμένα (P=0,009) και Συνολικά (P=0,005) Προβλήματα στα παιδιά στη δεύτερη μέτρηση. Το αν μίλησε η μητέρα στο παιδί για τη μαστεκτομή, σχετίζεται με Εσωτερικευμένα (P=0,014), Εξωτερικευμένα (P=0,050) και Συνολικά (P=0,010) Προβλήματα στα παιδιά, στην πρώτη μέτρηση και με Εσωτερικευμένα (P=0,017) στη δεύτερη. Συμπεράσματα: Μειονότητα των παιδιών παρουσίασε διαταραχές συναισθήματος και συμπεριφοράς (11,6% - 14,2% στην πρώτη αξιολόγηση και 9,4% - 18,1% στη δεύτερη), αλλά αυτά τα παιδιά μπορεί να χρειάζονται ψυχοκοινωνική υποστήριξη. Στις δύο μετρήσεις βρέθηκαν κοινοί παράγοντες να σχετίζονται με τις διαταραχές, αλλά μερικές φορές για διαφορετικό τύπο προβλήματος. Τα ευρήματα συμφωνούν με τη βιβλιογραφία και μπορούν να αποτελέσουν βάση για στρατηγικές πρόληψης και ανίχνευσης αναγκών παρέμβασης στο παιδί, στη μητέρα και στην οικογένεια. Χρειάζεται περαιτέρω έρευνα με τη συμμετοχή των παιδιών (άνω των 11 ετών) και του πατέρα, καθώς και μεγαλύτερη περίοδος μετα-παρακολούθησης.

Results: The sample's mean age was for children 11.1 ± 4.5 years and for mothers 44 ± 5.5 years. The 52.4% of children were boys. The 54.9% of mothers had undergone mastectomy. In both the first and second assessment, the majority of the participants, had talked on their own to the child about the disease (83.4% and 84.2%, respectively) and the mastectomy (60.7% and 65.5%, respectively). At the second assessment, was observed: increase (10.8%) in the information provided by mothers to child (regarding the disease), increase (3.9%) in children’s Internalizing Problems, decrease (2.2%) in children’s Externalizing Problems and increase (1.2%) in children’s Total Problems. Between the first and the second evaluation, children’s Internalizing Problems were more frequent (14.2% and 18.1%, respectively) than Externalizing (11.6% and 9.4%, respectively), while, there were no statistically significant changes in the mean value of the "CBCL" subscales. According to the results of multivariate analyses, the maternal Global Severity Index (SCL-90) is associated with children’s Internalizing (P=0.012), Externalizing (P=0.036) and Total Problems (P=0.001) on CBCL, at the first evaluation. The maternal Positive Symptom Total (SCL-90) is associated with children’s Internalizing Problems (P=0.018) at the second evaluation. The maternal Positive Approach (Ways of Coping) is negatively associated with children’s Internalizing Problems (P=0.046) at the first evaluation, while, the maternal Confrontive Coping (Ways of Coping) is associated with children’s Internalizing (P=0.001), Externalizing (P=0.000) and Total Problems (P=0.001) at the first evaluation and Externalizing Problems (P=0.031) at the second. The maternal Self-Sacrificing Style (DSQ) is associated with children’s Internalizing Problems (P=0.034), at the first evaluation, and Internalizing (P=0.030) and Total Problems (P=0.004) at the second. The maternal Maladaptive Style (DSQ) is associated with children’s Externalizing Problems (P=0.006) at the second evaluation. Higher scores on FAD-GFS are associated with higher children’s Internalizing (P=0.009) and Total Problems (P=0.005) at the second evaluation. Whether the mother had talked to the child, about the mastectomy, is associated with children’s Internalizing (P=0.014), Externalizing (P=0.050) and Total Problems (P=0.010) at the first evaluation and Internalizing Problems (P=0.017) at the second. Conclusions: A minority of children experienced emotional and behavioral disorders (11.6% - 14.2% in the first evaluation and 9.4% - 18.1% in the second), but these children may need psychosocial support. Same factors associated with the disorders in the two measurements were revealed, but sometimes for different types of problems. The findings of the study are consistent with the literature and can form the basis for preventive strategies and early detection of intervention needs of the child, the mother and the whole family. Further research involving the children themselves (over 11 years old) and the father is needed, as well as longer follow up.
