Σκοπός της παρούσας ανοιχτής, προοπτικής μελέτης είναι να αξιολογηθεί η επίδραση του πολυμορφισμού AluI του υποδοχέα της καλσιτονίνης (CTR) και του πολυμορφισμού BsmI του υποδοχέα της βιταμίνης D (VDR) στην ανταπόκριση της οστικής πυκνότητας στη χορήγηση κυκλικής ενδορινικής καλσιτονίνης 200IU σε μετεμμηνοπαυσιακές γυναίκες με οστεοπόρωση. ΥΛΙΚΟ ΚΑΙ ΜΕΘΟΔΟΣ : Στρατολογήθηκαν 169 μετεμμηνοπαυσιακές γυναίκες με οστεοπόρωση εκ των οποίων 157 γυναίκες έλαβαν για ένα χρόνο 200IU ενδορινικής καλσιτονίνης σε συνδυασμό με συνεχή χορήγηση 500mg στοιχειακού ασβεστίου και 0.25μg αλφακαλσιδόλης από του στόματος. Η οστική πυκνότητα της σπονδυλικής στήλης και του ισχίου (αυχένας μηριαίου κι τροχαντήρας) μετρήθηκαν στην έναρξη της θεραπείας και μετά από 12 μήνες. Συνελέγησαν στοιχεία από το ιστορικό όπως οι αρχικές μετρήσεις οστικής πυκνότητας που προσκόμισαν οι ασθενείς που πραγματοποίησαν μετεμμηνοπαυσιακά και πριν την έναρξη οιασδήποτε αγωγής για την οστεοπόρωση και το ατομικό ιστορικό καταγμάτων ...

Μετά από συγκατάθεση των ασθενών, ελήφθη δείγμα αίματος για γονιδιακό έλεγχο και απομόνωση του πολυμορφισμού AluI του υποδοχέα της καλσιτονίνης (CTR - AluI polymorphism) και οι πολυμορφισμού BsmI του υποδοχέα της βιταμίνης D (VDR - BsmI polymorphism). Η απομόνωση του DNA έγινε από τα λευκά αιμοσφαίρια ολικού αίματος με επξεργασία με τη μέθοδο της αλυσιδωτής αντίδρασης πολυμεράσης - PCR. Το δείγμα των ασθενών ήταν ομοιογενές, δηλαδή είχαν εμμηνόπαυση τουλάχιστον 8-10 χρόνια, δεν ήταν άνω των 80 ετών και δεν επελέγησαν ασθενείς που υπέστησαν εμμηνόπαυση κατόπιν ωοθηκεκτομής ή φαρμακευτικής αγωγής. Πριν την έναρξη της αγωγής, οι ασθενείς είχαν διακόψει άλλη αγωγή για την οστεοπόρωση (καλσιτονίνη, SERMs) τουλάχιστον ένα εξάμηνο. Οι ασθενείς δεν είχαν πάρει ποτέ διφωσφονικά ή αναβολική θεραπεία για την οστεοπόρωση (τεριπαρατίδη, παραθορμόνη) κατά το παρελθόν. Ηταν επιτρεπτό να έχουν λάβει προηγουμένως συμπλήρωμα ασβεστίου και αναλόγου της βιταμίνης D. Όλες οι ασθενείς ήταν Καυκάσιες και γενετικώς μη σχετιζόμενες μεταξύ τους. Το πρωτόκολλο της μελέτης εγκρίθηκε από την Επιτροπή Βιοηθικής και όλες οι ασθενείς υπέγραψαν οι ίδιες τη συγατάθεση συμμετοχής στη μελέτη. ............................................................................................

All women were Caucasian and genetically unrelated. Informed consent was obtained by all participant subjects. The protocol was approved by the Ethics Committee of Athens University. Past history of peripheral and vertebral fractures was recorded. RESULTS : Patients were divided in three subgroups according to the AluI polymorphism of the calcitonin receptor [TT n=73 (46%), TC, n=75 (48%), CC, n=9 (6%)] and three subgroups according to the BsmI polymorphism of the VDR receptor [BB n=42 (26%), Bb n=89 (56%), bb n=28 (18%)]. These results were in accordance to the results of other Caucasian populations. Baseline characteristics of participants, including age, body mass index and calcium intake were similar among groups. Baseline BMD of spine and hip showed no statistical difference among subgroups of polymorphisms although we noticed a lower spine and hip BMD value in bb subgroup and a slightly lower spine BMD in TT subgroup at the beginning of the study. There was an increase in spine BMD in all subgroups of CTR and VDR polymorphisms and an increase in hip BMD of TT and TC subgroups of the CTR polymorphism but these results were not statistically significant. Attempting to analyse the genotype combination of both CTR and VDR polymorphisms, we found that in some subgroups there were very few subjects that could not be analysed (e.g. CC subgroup) and in some combinations there were no patients. So, we analysed CC and TC subgroups together. We noticed lower spine and hip BMD values in bb+TT combination but with no statistical difference from other combinations. There was no statistical increase in BMD values in any genotype combination during the study. During the analysis of the preexisting vertebral and peripheral fractures, we noticed higher incidence of vertebral fractures in bb genotype and peripheral fractures in TT genotype, but none of these were statistically significant, perhaps due to the small number of subjects. During the study, no fractures were reported. CONCLUSION: All patients with different calcitonin and vitamin D receptor gene polymorphisms (AluI and BsmI) can benefit from the therapy with calcitonin. No subgroup showed statistical increase of the spine and hip BMD during the calcitonin therapy. Large numbers of patients are needed in order to show whether there is an impact of calcitonin therapy according to the CTR and VDR polymorphisms.
