Η λοίμωξη του κατώτερου γεννητικού συστήματος από τον ιό HPV αποτελεί μία από τις πλέον συχνές σεξουαλικά μεταδιδόμενες νόσους. Ο καρκίνος του τραχήλου της μήτρας, με βάση τα δεδομένα του Παγκόσμιου Οργανισμού Υγείας, είναι δεύτερος σε συχνότητα στον γυναικείο πληθυσμό. Σύμφωνα με τα δεδομένα της παγκόσμιας βιβλιογραφίας υπάρχει άμεση αιτιολογική συσχέτιση του καρκινώματος του τραχήλου της μήτρας με τους λεγόμενους ογκογόνους τύπους του HPV (Walboomers, Jacobs et al. 1999). Το γονιδίωμα του HPV έχει τυποποιηθεί στο 99.7% των γυναικών με καρκίνο του τραχήλου της μήτρας και πλέον θεωρείται αναγκαίος συμπαράγοντας για την ανάπτυξη του. Μολονότι έχουν ταυτοποιηθεί περισσότερα από 100 στελέχη του ιού (Longworth and Laimins 2004), πέντε κυρίως (16, 18, 31, 33, 45) ενοχοποιούνται για την ανάπτυξη των διηθητικών καρκινωμάτων του τραχήλου της μήτρας (Wright, Goldie et al. 2000). Σύμφωνα με τα μέχρι σήμερα δεδομένα, η εμφάνιση ενδοεπιθηλιακών αλλοιώσεων, στις οποίες ανιχνεύονται οι διάφοροι τύποι του HPV, προηγείται της ανάπτυξης καρκίνου τραχήλου της μήτρας. Είναι χαρακτηριστικό ότι οι γυναίκες με τύπους υψηλού κινδύνου του ιού έχουν σημαντικά αυξημένο κίνδυνο να αναπτύξουν καρκίνο του τραχήλου της μήτρας κατά τη διάρκεια της ζωής τους. Παρόλα αυτά, σημαντικό ποσοστό των ενδοεπιθηλιακών αλλοιώσεων συνήθως υποστρέφει ή δεν εξελίσσεται σε μεγαλύτερης βαρύτητας αλλοίωση. Μόνο στο 10-20% των γυναικών με ενδοεπιθηλιακή αλλοίωση του τραχήλου της μήτρας θα αναπτυχθεί καρκίνωμα ελλείψει αποτελεσματικής θεραπείας (Doorbar 2006). Οι διάφοροι τύποι του HPV κατατάσσονται και ορίζονται με βάση τα χαρακτηριστικά του γονιδιώματος τους. Ο μέχρι τώρα επίσημος ορισμός και κατηγοριοποίηση των τύπων του HPV γινόταν με κριτήρια υβριδοποίησης του DNA τους, όμως αυτή η μέθοδος έχει κάποια μειονεκτήματα και περιορισμούς. Έτσι για παράδειγμα κάποιοι HPV τύποι που με την τεχνική της υβριδοποίησης φαίνονταν να έχουν μικρές ή και καθόλου διαφορές, μπορεί να είχαν νουκλεοτιδική ομοιότητα μεγαλύτερη από 50% και αυτό εξαιτίας της διασποράς μικρών νουκλεοτιδικών ομολογιών. Για να ξεπεραστεί αυτή η ασυνέπεια, το γονιδίωμα του HPV ορίζεται ως καινούριος HPV τύπος όταν παρουσιάζει μεγαλύτερη του 10% διαφοροποίηση στις 10 συνδυασμένες νουκλεοτιδικές αλληλουχίες των γονιδίων E6, E7 και L1 όταν αυτές συγκριθούν με αλληλουχίες για τα ίδια γονίδια κάθε γνωστού τύπου. Τα γονίδια L1 και L2, τα οποία κωδικοποιούν τις αντίστοιχες πρωτεΐνες του καψιδίου του ιού, συνθέτουν το 83% και 17% του περιβλήματος του ιού αντίστοιχα. Αυτός είναι και ο λόγος για τον οποίο το γονίδιο L1 ανιχνεύεται από τα πρώτα στάδια της ιικής μόλυνσης. Έτσι η L1 πρωτεΐνη έχει χρησιμοποιηθεί ως ο ιδανικός στόχος του προφυλακτικού HPV εμβολίου, καθώς αυτή έχει την ιδιότητα να αναδιπλώνεται και να οργανώνεται σε σωματίδια που προσομοιάζουν τη δομή του καψιδίου του ιού (virus like partickles), τα οποία με τη σειρά τους είναι υπεύθυνα για την ενεργοποίηση της ανοσολογικής απάντησης του οργανισμού έναντι του ιού. Οποιεσδήποτε αλλοιώσεις οι οποίες μπορεί να εμπλέκονται με τη δομή, τις αντιγονικές ιδιότητες και τη λειτουργία της συγκεκριμένης πρωτεΐνης είναι πολύ σημαντικές. Για παράδειγμα η ποικιλομορφία του HPV 16 που φέρει το όνομα 114Κ αναδιπλώνεται σε σωματίδια που προσομοιάζουν τη δομή του καψιδίου του ιού (virus like partickles) με ένα ετερόλογο σύστημα έκφρασης, ενώ ο πρωτότυπος κλώνος της αλληλουχίας του HPV 16 δεν έχει την ίδια ιδιότητα. Αυτή η διαφορά αποδίδεται σε μια και μόνο αμινοξική διαφορά H228D της πρωτεΐνης L1. Άλλες διαφορές στην περιοχή L1 μπορεί να είναι σημαντικές για τη διάκριση των δυνατοτήτων επιμόλυνσης των διαφόρων παραλλαγών του HPV, καθώς και για τον ορισμό επιτόπων σχετικών με τη σχεδίαση του εμβολίου (Kirnbauer, Taub et al. 1993). Οι ογκοπρωτεΐνες Ε6, Ε7 παίζουν σημαντικό ρόλο στην HPV επαγόμενη καρκινογένεση του τραχήλου της μήτρας (Hildesheim, Schiffman et al. 2001). Η πρωτεΐνη Ε6 ενεργοποιεί σε μεταγραφικό επίπεδο την αποικοδόμηση του γονιδίου p53, επιταχύνοντας εντέλει τον κυτταρικό πολλαπλασιασμό. Η πρωτεΐνη Ε7 προσδένεται και απενεργοποιεί το γονίδιο pRb επάγοντας έτσι τον κυτταρικό κύκλο. Έχει επίσης αναφερθεί ότι συγκεκριμένες αλλαγές στην πρωτεΐνη Ε6 εμπλέκονται με την ανοσολογική απόκριση των κυτταροτοξικών T λεμφοκυττάρων (Matsumoto, Yasugi et al. 2003). Στην παρούσα μελέτη αναλύθηκαν για πρώτη φορά στον ελληνικό χώρο οι 5 πιο ογκογόνοι τύποι του ιού των ανθρώπινων θηλωμάτων σε μοριακό επίπεδο με βάση τα γονίδια L1, Ε6, Ε7 που εντοπίζονται στο γονιδίωμα του ιού. Με βάση τα γονίδια αυτά έγινε στατιστική επεξεργασία και φυλογενετική μελέτη με χρήση προγραμμάτων 11 βιοπληροφορικής, από τα οποία προέκυψαν οι υπότυποι και οι παραλλαγές (variants) του ιού, που κυκλοφορούν στον ελληνικό χώρο. Η φυλογενετική μελέτη έδειξε ότι οι απλότυποι των HPV τύπων, που απομονώθηκαν από γυναίκες που ανήκουν στην Ελληνική επικράτεια, κατατάσσονται σε μεγαλύτερο ποσοστό στον Ευρωπαϊκό και σε μικρότερο ποσοστό σε άλλους κλάδους (Αφρικανικό, Αμερικάνικο, Ασιατικό). Κατόπιν αναλύθηκαν σε νουκλεοτιδικό και σε αμινοξικό επίπεδο οι διαφορές των υποτύπων του ιού που βρέθηκαν. Τέλος αξιολογήθηκε με βάση τις βιβλιογραφικές αναφορές, αν αυτές οι διαφορές στο γονιδίωμα του ιού μπορεί να επιφέρουν αλλαγές στις βιολογικές του ιδιότητες οι οποίες μπορεί να σχετίζονται με την πρόκληση παθογένειας του ιού και τη δράση του εμβολίου. Πραγματοποιήθηκε επίσης ο συσχετισμός της παρουσίας ποικιλομορφιών του ιού HPV με αλλοιώσεις όλων των σταδίων του επιθηλίου του τραχήλου της μήτρας. Επιδημιολογικά δεδομένα που στηρίζονται σε μελέτες από όλο τον κόσμο δείχνουν ότι οι διάφορες παραλλαγές που ανήκουν στον ίδιο τύπο HPV, είναι βιολογικά διαφορετικές και μπορεί να έχουν διαφορετική παθολογική επικινδυνότητα. Η ανίχνευση των παραλλαγών του HPV και η ανάλυσή τους τόσο σε φυλογενετικό επίπεδο, όσο και σε επίπεδο πρωτεϊνικής έκφρασης μπορεί να κατέχει σημαίνοντα ρόλο στην εξέλιξη του μεταστατικού καρκίνου του τραχήλου της μήτρας. Η ανάλυση αυτή μπορεί να αποδειχθεί σημαντική για την πρόβλεψη της παθογένειας του ιού, για τη σχεδίαση κατάλληλων στρατηγικών διάγνωσης και για τη βελτίωση του εμβολίου για τον HPV.

The objectives of the present study were firstly to characterize the E6 and E7 intratype genetic variability of HPV 18, 16, 31, 33, 45 variants, which are the most common types of high grade lesions in Greek women and correlate these results with the sequence variation in L1 gene of the same samples, and secondly, to investigate a possible correlation of specific variants or lineages with cytological diagnosis. The nomenclature and taxonomy of molecular variants has not been determined by the International Committee on Viral Taxonomy. The majority (93%) of the HPV 16 samples examined in this study belong to the European lineage and only a small percentage is assigned to the non European lineage. Many studies show a relation of HPV 16 genome variations with increased risk for cervical neoplasia. The European variants E (B1) and E (B1039) gather the majority (93%) of samples with high grade lesions and squamous cell carcinoma. The two new E6 variants (G176C and C245T) resulted in amino acid variations D25H and R48W, which are associated with HGSIL and LGSIL lesions and belong to European lineage. The amino acid polymorphism L83V of E6 oncogene, was found in a large number of samples associated with high grade lesions that belong to European and Asian American lineages. This polymorphism may give to HPV higher carcinogenic potential due to the fact that it is located within the epitope which binds to MHC molecules and appears to enhance MAPK signaling. The amino acid changes R10I, Q14D and Q14H have been described before and are under strong diversifying or Darwinian selection. 221 These changes are found on African and Asian American lineages and associated mainly with HGSIL lesions. For E7 gene of HPV 16, higher conservation was observed, especially among the European variants and only one amino acid change was detected (nt 647, N29S) on samples of African lineage which are associated mostly with HGSIL lesions. E7 gene of HPV 16 seems to be conserved, as only five nucleotide changes were detected, mostly in the non European variants. The lack of heterogeneity of HPV 16 E7, shows high evolutionary purifying selection and might be related to its oncogenic potential. HPV 31 variants have been classified as lineages A, B, C, based on whole genome analysis. The phylogenetic analysis of combined genes E6, E7, L1 revealed that the HPV 31 variants detected in Greece are clustered into all the three different lineages. The lineage C gathers most of the isolates examined. In comparison to C clade, lineages A and B have a lower divergence between them and fewer variants. It is worth to mention that 24 of the 36 samples are identical to variants C (B1230) which seems to be the most prevalent (66%) and most distant to the reference sequence (lineage A). The phylogenetic results of combined genes confirm those that have been published for L1 gene. Variants are clusterd in the same lineages and the C variant (HM596540) of L1 gathers most of the isolates studied. Even though HPV 31 is considered the fourth most common HPV type in squamous cervical carcinomas, the published reports regarding the variant-related pathogenesis of cervical lesions are rare and most of them show a lack of association between cervical lesion of HPV 31 and polymorphisms detected. Our data indicate that among Greek women with HPV 31 infections, the increased risk of Low grade and High grade lesions was associated with C lineage. In the present study the molecular analysis detected high nucleotide and amino acid variation. The amino acid site Y23H is important as it interacts with pRb protein. The new silent nucleotide substitutions C (A781T) is found at LGSIL lesions. Even though the HPV 31 and HPV 33 are phylogenetically closely related to HPV 16, the geographic distribution of variant lineages of these types is not yet well established and limited information is available about the genomic diversity. Limited variation was detected among the HPV 33 sequences studied. The new variant A1 (A138C) detected in E6 gene, is associated with HGSIL lesions. Also three and two nucleotide substitutions were found among the E6 and E7 variants respectively, which have been described before. The dichotomy detected among HPV 33 subtypes, could 222 be an evolutionary process that may lead to new HPV subtypes and eventually new types. It seems that the analysis of L1 gene of HPV 33 was not informative enough, since the phylogenetic tree that emerged is not similar the combined analysis of genes. This contradiction may be due to the limited nucleotide polymorphism detected. The combined genomic regions indicate that the HPV 33 variants occurring in Greece, are clustered with the reference sequence in Lineage A and none of them belongs to Lineage B. All the samples with HGSIL lesions are clustered in A1 clade and have more similarities with the reference sequence in comparison to clade A2. The E6, E7, and E1 genomic regions of four HPV 18 Greek isolates were combined with the L1 region. The phylogenetic analysis of combined sequences of these regions is more informative in comparison to the analysis of the L1 gene alone, since apart from the lineages, sublineages have emerged. HPV 18 variants have co-evolved with the three major human branches and were clustered as European (E), Asian–American (AA), and African (Af) lineages. Oncogenic potential of HPV 18 varies geographically and the reason may be the molecular variants that prevail in specific regions. Most of the isolates occurring in Greece belong to the European Lineage (E2) and are associated with LGSIL lesions. Noucleotide analysis of E6 and E7 revealed two variants with four amino acid alterations, none of which is new. The amino acid variations H80Y, N129K found at E6 gene and H2Y, N29S found at E7 gene, are detected in Af variants. Codon 129 is highly conserved within oncogenic HPV genotypes and the amino acid change N129K detected in many HPV 18 Af variants. We could not observe a significant correlation between HPV 18 variants found in this study and the results from cervical cytopathologic tests, due the small number of samples analyzed. HPV 18 and HPV 45 account for approximately 20% of all cervix cancers. However the evolutionary history and sequence variability of HPV 45 have not been studied extensively. In this study four variants have emerged from the combined analysis of genes. From the two variants of E6 and E7 genes, one is new A2 (T245C) and is associated with WNL cytologic samples. It seems that L1 gene is more informative for HPV 45, since two more variants were detected for this gene. There is a dichotomy in the tree formed from the combined genes and most of the isolates studied are classified in lineage B1 which is distant from the reference sequence. None of the isolates is identical to the reference sequence and only one is clustered at Lineage A2. Lineage B1 consists of samples with LGSIL and SCC cytological results. 223 The present research concerns apart from the HPV 16 type, which is the most prevalent, other HPV types that are less studied in worldwide level. A wide range of cytological lesions was included and some of them are associated with specific phylogenetic clades. Finally for the first time in Greece the clades that emerged for each HPV type are correlated with geographic data.
