ΕΙΣΑΓΩΓΗ: Η αύξηση της αρτηριακής πίεσης (ΑΠ) κατά την πρωινή έγερση μετά την αφύπνιση (morning blood pressure surge - MBPS) έχει συσχετιστεί με αυξημένο καρδιαγγειακό κίνδυνο. Διάφοροι ορισμοί έχουν χρησιμοποιηθεί στη βιβλιογραφία, χωρίς να έχει γίνει σαφές ποιος είναι ο βέλτιστος ορισμός για τον υπολογισμό της MBPS. Προκειμένου να εκτιμηθεί η αξιοπιστία των διαφόρων ορισμών, εξετάστηκε η επαναληψιμότητά (ΕΠΨ) τους με δύο τρόπους.ΣΚΕΛΟΣ Α. Αποτελέσματα: ορισμοί MS1-5 CCC: 0.20/0.30, 0.43/0.45, 0.53/0.51, 0.51/0.47, 0.46/0.48 (συστολική/διαστολική πίεση), SDD 14.3/11.4, 12.1/9.9, 11.2/9.5, 10.3/8.2, 11.9/9.8, CV 0.49/0.57, 0.44/0.39, 0.37/0.35, 0.36/0.31, 0.27/0.24 και η συμφωνία στην ανίχνευση των surgers 69%/70%, 71%/76%, 75%/75%, 81%/83%, 74%/75% με συντελεστή kappa 0.18/0.20, 0.23/0.36, 0.33/0.33, 0.49/0.53 και 0.29/0.31. ΣΚΕΛΟΣ Β. Μέθοδος και άτομα: Η βραχυπρόθεσμη ΕΠΨ μέσα στο ίδιο 24ωρο εξετάστηκε στα άτομα με απογευματινό ύπνο (siesta) κατά τη διάρκεια της ABPM, συγκρίνοντας την πρωινή με την απογευματινή αύξηση της ΑΠ (evening surge - ES) μετά τη siesta, με βάση 4 διαφορετικούς ορισμούς κατά αντιστοιχία των ορισμών MS1,2,4,5. Αποτελέσματα: 562 ABPM (476 άτομα, μέση ηλικία 54.9±13.2 έτη, 47% υπό θεραπεία). Η ΕΠΨ μέσα στην ίδια ABPM (intraindividual reproducibility) ήταν φτωχή. Παρατηρήθηκε περίπου 70% συμφωνία μεταξύ MBPS και ES στην ανίχνευση των surgers (kappa 0.18).ΣΥΜΠΕΡΑΣΜΑ: Ο ορισμός MS4 φαίνεται να παρουσιάζει την καλύτερη ΕΠΨ στην εκτίμηση της πρωινής αύξησης. Αν και η ΕΠΨ στην ίδια ABPM είναι φτωχή, περίπου 70% των ατόμων που ήταν surgers το πρωί, ήταν surgers και το απόγευμα. Επομένως, η αύξηση της ΑΠ μετά την έγερση φαίνεται να αποτελεί ενδογενές χαρακτηριστικό της διακύμανσης της ΑΠ του κάθε ατόμου.

Results: CCC was 0.20/0.30, 0.43/0.45, 0.53/0.51, 0.51/0.47, 0.46/0.48 (systolic/diastolic) for MS1-5 respectively, SDD 14.3/11.4, 12.1/9.9, 11.2/9.5, 10.3/8.2, 11.9/9.8 respectively, CV 0.49/0.57, 0.44/0.39, 0.37/0.35, 0.36/0.31, 0.27/0.24 and agreement in detecting surgers 69%/70%, 71%/76%, 75%/75%, 81%/83%, 74%/75% with kappa 0.18/0.20, 0.23/0.36, 0.33/0.33, 0.49/0.53 and 0.29/0.31 respectively. PART B. Subjects and methods: Short-term intraindividual reproducibility within the same 24-hour period was tested in subjects with a siesta during ABPM by assessing the MS versus the evening surge (ES) after siesta. MS and ES were assessed using four different definitions. The intraindividual reproducibility was assessed using the SDD between MS and ES, the CCC, the CV and the agreement between MS and ES in detecting “surgers”. A total of 562 ABPM were analyzed (476 subjects, mean age 54.9±13.2 years, treated 47%). Average MS (16.3/14.4 mmHg, systolic/diastolic) was higher than ES (13.3/12.1 mmHg, p<0.001) due to higher post-rising BP in the morning (p<0.01). The intraindividual reproducibility was rather poor with no clear differences among different definitions. There was about 70% agreement between MS and ES in the detection of “surgers” (systolic and diastolic, kappa statistic 0.18). CONCLUSION: MS4 appears to provide the most reproducible definition of MS. Furthermore, although the intraindividual reproducibility of the BP surge within the same 24-hour period is rather poor, about 70% of the “morning surgers” are also “evening surgers”. Thus, the BP surge appears to be an intrinsic pathophysiologic characteristic of the BP behavior of an individual.
