Οι μορφογενετικές πρωτεΐνες των οστών (BMPs) ανήκουν στην υπερ-οικογένεια του μετατρεπτικού αυξητικού παράγοντα-β (TGF-β). Μέχρι σήμερα έχουν ταυτοποιηθεί τουλάχιστον 20 από αυτές. Οι BMPs συνδεόμενες με τους αντίστοιχους υποδοχείς BMPRIa, BMPRIb και BMPRIΙ, σχηματίζουν ετερομερή σύμπλοκα του τύπου της σερίνης/θρεονίνης κινάσης. Αυτά με τη σειρά τους φωσφορυλιώνουν συγκεκριμένα μέλη της οικογένειας των Smad (-1, -5, -8), τα οποία μεταφέρονται στον πυρήνα, όπου ενεργοποιούν πολλά γονίδια-στόχους, υπεύθυνα για συγκεκριμένες λειτουργίες του κυττάρου. Με τον τρόπο αυτό, οι BMPs διαδραματίζουν σημαντικό ρόλο στη διαφοροποίηση, τον πολλαπλασιασμό, την απόπτωση και την επιβίωση πολλών κυττάρων (φυσιολογικών, αλλά και νεοπλασματικών). Είναι λοιπόν φανερό ότι η επίδρασή τους αφορά τόσο την εμβρυογένεση – οργανογένεση, όσο και τη συνήθη λειτουργία των φυσιολογικών κυττάρων κατά την ενήλικη ζωή, αλλά και την ογκογένεση ή τη συμπεριφορά των κυττάρων σε παθολογικές, μη νεοπλασματικές, καταστάσεις. Η παρούσα μελέτη έχει ως σκοπό να διερευνήσει την έκφραση των BMPs σε νεοπλάσματα που διαπιστώθηκε έκφρασή τους σε πιλοτική μελέτη, χωρίς αντίστοιχα βιβλιογραφικά δεδομένα, στο Κεντρικό Νευρικό Σύστημα, στο Σύστημα των παραγαγγλίων και στο Γαστρεντερικό Σύστημα. Οι δύο πρώτες ομάδες περιελάμβαναν 7 γλοιοβλαστώματα, 12 παραγαγγλοιώματα, και 5 φαιοχρωμοκυτώματα. Η τρίτη ομάδα που αφορούσε στο Γαστρεντερικό Σύστημα περιελάμβανε 29 αδενοκαρκινώματα στομάχου, 12 διάχυτου και 17 εντερικού τύπου, καθώς και 37 αδενοκαρκινώματα παχέος εντέρου, 10 καλής, 22 μέτριας και 5 χαμηλής διαφοροποίησης. Η διερεύνηση έγινε με την μέθοδο της ανοσοϊστοχημείας για τα BMPs (-2, -4, -6 και -7) και τους υποδοχείς τους (BMPRIa και BMPRIb), σε τομές παραφίνης. Χρησιμοποιήθηκαν τα πολυκλωνικά αντισώματα της Santa Cruz και εφαρμόστηκε η μέθοδος στρεπταβιδίνης–βιοτίνης. Οι ιστικές τομές αξιολογήθηκαν για την ένταση και την έκταση των ανοσοχρώσεων. Η τελική επεξεργασία των αποτελεσμάτων και η εξαγωγή των στατιστικών στοιχείων έγινε με την βοήθεια του στατιστικού πακέτου SPSS.11.0. Στην παρούσα μελέτη διαπιστώσαμε ότι τόσο τα BMPs όσο και οι υποδοχείς τους εκφράζονται σε όλα τα νεοπλάσματα τα οποία αποτέλεσαν υλικό της έρευνας καθώς και στους φυσιολογικούς ιστούς από τους οποίους προέρχονται, με ποικίλη ένταση και έκταση. Ειδικότερα: • Οι BMPs και οι υποδοχείς τους εκφράζονται τόσο στα παραγαγγλιώματα, όσο και στα φαιοχρωμοκυτώματα, αλλά με ουσιώδεις διαφορές μεταξύ τους. Οι διαφορές αυτές επιτρέπουν την ανοσοϊστοχημική διάκριση μεταξύ των δύο όγκων αφού η BMP-6 και οι υποδοχείς BMPRIa και BMPRIb εκφράζονται εντονότερα και πιο εκτεταμένα στα φαιοχρωμοκυτώματα. • Στα γλοιοβλαστώματα αντίθετα, καταδεικνύεται η χαμηλή έκφραση της BMP-6 και του υποδοχέα BMPRIa. • Οι BMPs και οι υποδοχείς τους εκφράζονται τόσο σε αδενοκαρκινώματα στομάχου όσο και σε αδενοκαρκινώματα παχέος εντέρου με αρκετές διαφορές μεταξύ τους και μεταξύ των υποδιαιρέσεών τους με βάση τον ιστολογικό τύπο ή το βαθμό διαφοροποίησης. Στα αδενοκαρκινώματα στομάχου η BMP-6 εκφράζεται σε μεγαλύτερες ποσότητες στα κύτταρα των νεοπλασμάτων εντερικού τύπου απ’ ό,τι στα αντίστοιχα διαχύτου τύπου. Επιπλέον, υπάρχουν στατιστικά σημαντικές διαφορές στην έκφραση της BMP-6 ανάμεσα στα νεοπλασματικά κύτταρα των πρωτοπαθών και των μεταστατικών εστιών των αδενοκαρκινωμάτων του στομάχου. • Τα ευρήματά μας τόσο στα παραγαγγλιώματα–φαιοχρωμοκυτώματα όσο και στα αδενοκαρκινώματα στομάχου-παχέος εντέρου καταδεικνύουν την διαφορετικότητα του πρωτεϊνικού προφίλ νεοπλασμάτων παρόμοιας καταγωγής, αλλά διαφορετικής εντόπισης, που για τα πρώτα, θα μπορούσε επιπροσθέτως να βοηθήσει στην ιστολογική διάγνωση τους. Περαιτέρω έρευνα στην κατεύθυνση αυτή θα διευρύνει την κατανόηση των μηχανισμών ανάπτυξης αυτών των νεοπλασμάτων, που είναι προϋπόθεση στοχευμένων θεραπευτικών προσπαθειών.

They are also involved in a variety of biological effects which are related to homeostasis and chemotaxis, like the migration of cells, or their activation in emergency situations. There is evidence that the BMPs are implicated in several aspects of embryonic development, from the establishment of the basic embryonic body plan to the morphogenesis of several organs, but also in the regular functions of normal cells during adulthood. Recent studies have demonstrated the involvement of BMPs and their receptors in the development of a variety of pathologic conditions, including several human tumors, such as carcinomas. In this study, we have investigated the expression of BMPs and their receptors in some neoplasms, for which there were no data in the literature. Following a pilot study of various tumors, we decided to include neoplasms of the central nervous system (glioblastomas, n=7), of the system of paraganglia (paragangliomas, n=12, and pheochromocytomas, n=5), and of the gastrointestinal system (gastric adenocarcinomas, n=29, and adenocarcinomas of large bowel, n=37). Twelve gastric adenocarcinomas were of the diffuse, and 17 of the intestinal type. Ten intestinal adenocarcinomas were welldifferentiated, 22 moderately differentiated and 5 poorly differentiated. Paraffin blocks of all the specimens were retrieved from the archives, and 3μ-thick sections were prepared for immunohistochemistry. We used commercially available goat polyclonal antibodies for BMPs (-2, -4, -6 and -7) and for their receptors (BMPRIa, BMPRIb), purchased from Santa Cruz Biotechnology (Santa Cruz, California, USA). Immunohistochemical stains were performed by a standard streptavidin-biotin method. On microscopic examination, both the intensity and the extent of the immunohistochemical expression of BMPs and their receptors were graded semiquantitatively. Statistics were performed with the SPSS 11.0 software. The BMPs and their receptors were expressed in all the neoplasms evaluated, as well as in the normal tissues from which they arose. The intensity and the extent of immunoreactivity were variable. More specifically: - BMPs and their receptors were expressed in both paragangliomas and pheochromocytomas, but there were differences between the two types of tumors (BMP-6 and the receptors BMPRIa and BMPRIb were expressed more intensely and extensively in pheochromocytomas). Such differences may be useful in distinguishing these two tumor types. - The glioblastomas were characterized by low expression of BMP-6 and of the receptor BMPRIa. - BMPs and their receptors were expressed both in gastric adenocarcinomas and in adenocarcinomas of the large bowel. However, there were differences between the adenocarcinomas of these two locations; there were also differences between the subtypes of gastric cancer (diffuse versus intestinal), and among the grades of differentiation of intestinal cancer. A prominent difference concerned BMP-6, which was expressed in larger quantities in the intestinal type of gastric adenocarcinoma than in the diffuse type. In addition, there were statistically significant differences in the expression of BMP-6 between the neoplastic cells of the primary tumors and those of the nodal metastases of gastricadenocarcinomas. - Our findings regarding the paragangliomas and the pheochromocytomas, as well as the adenocarcinomas of two different parts of the gastrointestinal tract indicate the presence of differences in protein profile between histologically similar tumors. Further research in this direction will enhance the understanding of the mechanisms of development of these neoplasms, which is a prerequisite for targeted therapeutic efforts.
