Η ανθρώπινη πρωτεΐνη KCNE1 σχηματίζει την β-υποομάδα των διαύλων Iκs καλίου και παίζει σημαντικό ρόλο στην ρύθμιση της διάρκειας του κολπικού δυναμικού ενέργειας. Στην μελέτη αυτή εκτιμήσαμε την σχέση μεταξύ της μη συνώνυμης μετάλλαξης 112G>A του γονιδίου KCNE1 και της εμφάνισης κολπικής μαρμαρυγής μετά από επεμβάσεις ανοικτής καρδιάς.ΑΣΘΕΝΕΙΣ – ΜΕΘΟΔΟΙ – ΑΠΟΤΕΛΕΣΜΑΤΑΜελετήσαμε σε προοπτική βάση 509 διαδοχικούς ασθενείς με καρδιοπάθεια που υποβλήθηκαν σε εγχείρηση ανοικτής καρδιάς. Σε 203 ασθενείς εμφανίστηκε κολπική μαρμαρυγή μετά την καρδιοχειρουργική επέμβαση. Ο ατομικός σχετικός κίνδυνος εμφάνισης μετεγχειρητικής κολπικής μαρμαρυγής αυξήθηκε καθώς αυξανόταν ο αριθμός των αλληλόμορφων G από 1.36 (95% διαστήματα αξιοπιστίας [CI] 0.89-2.08) για ετεροζυγώτες του αλληλόμορφου G σε 1.62 (95% CI 1.08-2.43) για ομοζυγώτες του αλληλόμορφου G (p=0.04). Στην πολυπαραγοντική ανάλυση το μειωμένο κλάσμα εξώθησης της αριστερής κοιλίας (αναλογία πιθανοτήτων, Odds Ratio [OR] 1.585, 95% CI 1.076-2.331, p=0.020), η ηλικία (OR 1.043, 95%CI 1.022-1.064, p<0.001), ο τύπος της χειρουργικής παρέμβασης (αντικατάσταση της αορτικής βαλβίδας) (OR 1.869, 95%CI 1.094-3.194, p=0.022) και ο γονότυπος 112G>A (OR 1.401 [σε αθροιστικό μοντέλο], 95%CI 1.052-1.865, p=0.021) ήταν ανεξάρτητοι προγνωστικοί παράγοντες για την εμφάνιση κολπικής μαρμαρυγής μετά επεμβάσεις ανοικτής καρδιάς. ΣΥΜΠΕΡΑΣΜΑΤΑ Η μελέτη αυτή επιβεβαίωσε την συσχέτιση του πολυμορφισμού 112G>A και της μετεγχειρητικής κολπικής μαρμαρυγής σε ασθενείς που υποβάλλονται σε επέμβαση ανοικτής καρδιάς.

The KCNQ1 gene codes for the α-subunit of the Iks channel and it was the first to be related to familial AF.Mutations in the KCNQ1 gene have led to further studies on KCNE1 gene, as a possible target for genetic predisposition to AF. The KCNE1 gene forms the β-subunit of the potassium channel carrying the cardiac Iks current. Cardiac Iks channels contribute to atrial repolarization in the late phase of the action potential. Cardiac Iks channels are capable of modulating the action potential duration and restitution properties of the atrial tissue as part of their contribution in the slowly activating delayed rectifier potassium current. A nonsynonymous, single-nucleotide polymorphism (SNP) in the KCNE1 gene (A to G substitution at position 112, rs1805127), leading to a glycine substitution for serine at amino-acid position 38 (S38G), was previously associated with increased AF incidence. Of note, the S38G variant in the KCNE1 gene reduces Iks current density and prolongs the atrial action potential duration, whereas familial AF-causingmutations in Iks channel genes have all had gain-of function effects.In the present study, we assessed the impact of the KNCE1 S38G polymorphism on the incidence of postoperative AF in a prospectively recruited cohort of patients subjected to cardiac surgery. To the best of our knowledge, no data are available on the effect of the KCNE1 polymorphisms to postoperative AF.Methods and resultsA cohort of patients scheduled to undergo open heart surgery was prospectively recruited. Blood samples were drawn and patient were followed for development of AF post operatively until discharge. Genetic analysis was performed using PCR, Sequencing and Restriction Fragment Length Polymporphysm (RFLP). In toatl 509 patients were recruited of whom 203 (39.9%) had at least one qualifying episode of post operative AF. Higher frequency of the G allele was observed in the post operative AF group compared to the control group (0.628 vs 0.552, respectively, P = .016). According to the study findings the individual’s realtive risk for postoperative AF increased as the number of G allele increased from 1.36 (95% confidence interval [CI] 0.89-2.08) for G allele heterozygotes to 1.62 (95% confidence interval [CI] 1.08-2.43) for G allele homozygotes (P = .04).Multivariate stepwise binary regression revealed the abnormal ejection fraction (odds ratio [OR] 1.585, 95% CI 1.076-2.331, P = .020), age (OR 1.043, 95% CI 1.022-1.064, P b .001), type of surgery (aortic valve replacement) (OR 1.869, 95% CI 1.094-3.194, P = .022), and the 112GNA genotype (OR 1.401 [in additive model], 95% CI 1.052-1.865, P = .021) to be independent predictors of postoperative AF. DiscussionThis is the first study to evaluate the effect of KCNE1 gene polymorphism in the development of post operative atrial fibrillation. Our study results showed an association between the number of G allele and the risk of postoperative AF. Patients with two copies of G allele exhibited 60% higher risk of developing post operative AF when compared to patients with AA genotype. Furthermore, multivariate stepwise binary regression showed this effect to be independent of patient or procedure related factors, including perioperative medications (β-blockers, ACE inhibitors, Statins) and the type of procedure. Our findings are in accordance with previous epidemiological observations, showing an association between 112G>A polymorphism and AF in general population and functional studies demonstrating an important impact of 112G>A polymorphism on the regulation of Iks current.
