Τα διάμεσα κύτταρα του Cajal παρέχουν μυογενή έλεγχο στην κινητικότητα του γαστρεντερικού συστήματος με την έναρξη των βραδέων κυμάτων στο στόμαχο, το έντερο και το κόλον ενώ θεωρούνται απαραίτητα και για τη νευροδιαβίβαση. Ανευρίσκονται στο μυεντερικό πλέγμα στον απώτερο στόμαχο, στο λεπτό και παχύ έντερο, στο εν τω βάθει μυϊκό πλέγμα του λεπτού εντέρου και στο υποβλεννογόνιο πλέγμα του λεπτού και παχέος εντέρου, όπως επίσης και μέσα στην κυκλοτερή μυϊκή στιβάδα. Είναι μικρά, τριγωνικά κύτταρα με μακριές προσεκβολές σε στενή σχέση με τα γειτονικά διάμεσα κύτταρα, τους εντερικούς νευρώνες ή τα μυϊκά κύτταρα. Πρόσφατες έρευνες έδειξαν ότι ο c-kit υποδοχέας είναι σημαντικός για την ανάπτυξη των διαμέσων κυττάρων. Πρόδρομα μεσεγχυματικά διάμεσα κύτταρα που φέρουν τον υποδοχέα c-kit χρειάζονται το σύνδεμα kit το οποίο προέρχεται από τα νευρικά ή λεία μυϊκά κύτταρα. Ανάλογα με την επίδραση του συνδέματος kit από τα νευρικά ή λεία μυϊκά κύτταρα, τα διάμεσα κύτταρα θα αναπτυχθούν είτε ως διάμεσα κύτταρα του μυεντερικού πλέγματος είτε ως ενδομυϊκά. Τα κύτταρα αυτά είναι σημαντικά στην ρύθμιση των επικοινωνιών μεταξύ των νευρικών και λείων μυϊκών κυττάρων. Μείωση του αριθμού των διαμέσων κυττάρων έχει αναφερθεί σε πολλές διαταραχές της κινητικότητας του γαστρεντερικού συστήματος στον άνθρωπο, συμπεριλαμβανομένων της νόσου του Hirschsprung, της υπογαγγλιόνωσης, της χρόνιας ιδιοπαθούς ψευδοαπόφραξης, της βραδείας διέλευσης δυσκοιλιότητας και της νεογνικής υπερτροφικής πυλωρικής στένωσης. Στη μελέτη μας εξετάσαμε την υπόθεση κατά πόσο η εντερική ισχαιμία διαφορετικής διάρκειας και η επαναιμάτωση, έχουν σημαντικές και καταστροφικές αλά αναστρέψιμες επιπτώσεις στα διάμεσα κύτταρα και στο κινητικό μοντέλο του λεπτού και παχέος εντέρου. Το υλικό ήταν ώριμοι επίμυες βάρους μεγαλυτέρου των 150gr και από τα δύο φύλα. Εκτέθηκαν αρχικά σε αιθέρα μέχρι να κατασταλεί μόλις η αναπνευστική λειτουργία και ακολούθως σε κεταμίνη και ξυλαζίνη για την επίτευξη πλήρους και μόνιμης αναισθησίας. Τα χειρουργικά βήματα ήταν η διάνοιξη της περιτοναϊκής κοιλότητας και η ανεύρεση της κοιλιακής αορτής ακριβώς εμπρός από την άνω μεσεντέριο αρτηρία. Στο σημείο αυτό τοποθετούνταν μια αγγειακή λαβίδα για την πρόκληση ισχαιμίας στο λεπτό και παχύ έντερο. Έγιναν τρεις ομάδες ισχαιμίας διάρκειας 15, 30 και 45 λεπτών. Σε κάθε ομάδα γινόταν λήψη ενός τμήματος από τη νήστιδα, τον ειλεό και το κατιόν κόλον αμέσως μετά την άρση της ισχαιμίας, 30 λεπτά, 60 λεπτά, 6 και 24 ώρες αργότερα. Τα ιστοτεμάχια τοποθετούνταν σε διάλυμα φορμαλδεΰδης και με τη χρήση του πολυκλωνικού αντισώματος CD117 γινόταν η αναγνώριση και μελέτη των διαμέσων κυττάρων.

Mesenchymal interstitial cells precursors that carry the c-kit receptor require the kit ligand, which can be provided by neuronal cells or smooth muscle cells. According to the influence of the kit ligand from either neuronal or smooth muscle cells, the ICC’s develop as either myenteric or intramuscular interstitial cells. These cells are important in modulating communications between nerve and muscle. A decrease in the number of ICC’s has been reported in several human gastrointestinal motility disorders, including Hirschsprung disease, hypoganglionosis, chronic idiopathic pseudoobstruction, slow transit constipation and neonatal hypertrophic pyloric stenosis. In this study we examined the hypothesis that the various duration ischemia and reperfusion have deleterious but reversible results on the ICC’s and in the motility model of the intestine and colon. Mature mice (>150 gm) of either sex were exposed initially to ether until breathing just ceased and in ketamine and xylazine for more permanent anesthesia. The surgical steps were the opening of the peritoneal cavity and the identification of the abdominal aorta just ahead (or above) the superior mesenteric artery. In this point we put a clamp to create ischemia in the small intestine and colon. There were three groups of ischemia 15, 30 and 45 min. In every group we take a small segment of jejunum, ileum and descending colon just after the removal of the clamp, 30 min, 60 min, 6 hours and 24 hours later. The specimens were put in formaldeide and the CD117 was used for the identification of the ICC’s. The results were that even short duration ischemia disrupts the ICC’s network in the gastrointestinal tract, and the damages are directly proportional to the duration of ischemia. The reperfusion has also a significant role in the changes of the cells and its network. From the different parts of the intestine the colon seems to be the most sensitive and the jejunum the most resistant in ischemia. The interstitial cells of Cajal seem to show plasticity, since their network even totally disrupted from ischemia in a few days regenerates especially the ICC’s network in the colon. For the intestine and colon motility it is not absolutely necessary the regeneration of all the ICC’s network, as the first intestinal movements appear even when the network has not been completely regenerated.
